Provide relationship-centred care management to support client wellbeing and independence at home. Duties include care planning, funding utilisation, client education, coordinating with clinical and allied health teams, identifying high-risk clients, initiating case conferences, promoting services, ensuring accurate billing and documentation, and following Aged Care Standards. Applicants need knowledge of Aged Care legislation and quality standards, understanding of ageing, case management experience, community services experience, independence, teamwork and strong digital literacy. This is a full-time fixed-term parental leave contract with work-from-home options and client visits across the Nepean region.
