Provide relationship-centred care management through a clinical lens, supporting client outcomes, wellbeing and safe independent living at home. Duties include care planning, clinical assessments, initial direct nursing care within scope, client education, risk monitoring, case conferencing, team coordination, service promotion, billing and documentation, and compliance with Aged Care Standards. Applicants need nursing or healthcare qualifications, current AHPRA Registered Nurse registration, aged care legislation and quality standards knowledge, strong clinical knowledge, case management experience, community nursing experience, teamwork skills and digital literacy. This is a full-time fixed-term parental leave contract with work-from-home options and client visits across the Nepean region.
