Training course: 12STEPS+Cause tree

This industry-leading RCA training course and methodology is a flexible, all-industry approach compatible with systemic approaches such as 6Sigma. It uses risk management and escalation techniques and includes a toolkit for solving highly complex and high-risk problems. The course covers Focus, Find Causes and Fix Forever, followed by practical work on a current unsolved organisational problem requiring RCA. Workshop format is two days. Online format is six sessions of 2.5 hours each.

Learn when and how to apply 5WHYS, expand it with teamwork, and develop skills in risk management, escalation, data collection, data analysis and action management. Learn to design Ishikawa or Fishbone diagrams, work with teams using RCA2GO software, present an A3 case and influence change. Numerous case studies are included. Workshop and online formats are available, both lasting one day.

Students are introduced to the origins of 5WHYS, its use in Toyota and the ideas of Nobel prize winner Daniel Kahneman that explain reasons behind the tremendous power of 5WHYS. Learn to identify unresolved issues, create a logical 5WHYS, test its flow, understand the importance of evidence, evaluate side effects and assign actions. Numerous case studies are included, and RCA2GO software is used throughout the training. Workshop and online formats are available, both lasting half a day.
