Eligible residents can use government-funded Support at Home services or an existing Home Care Package to help pay for care at Hawthorn Grange, including personal care, nursing and allied health support. Existing funded home care packages can be transferred to provide 24/7 support, with no reassessment or extra aged care fees required.
