Part time permanent role, minimum 3 days with flexible days and hours, travelling around Bathurst and surrounds. Provide Occupational Therapy assessments and interventions in clients' homes, including home assessments, activities of daily living advice, home modifications, equipment prescription, care planning and education. Facilitate community Health and Wellness programs, develop person-centred care plans, support community workers, work with the wider allied health team and liaise with external agencies and suppliers. Applicants need a tertiary Occupational Therapy qualification, post graduate experience, current AHPRA registration, experience with equipment prescription, pressure care, home modifications and falls prevention, a current driver's licence and registered and insured vehicle, and willingness to apply for a Police Check.
