Provide relationship-centred care management through a clinical lens, supporting client outcomes, wellbeing and safe independent living at home. Duties include care planning, client education, clinical assessments, direct nursing care within scope, risk screening, case conferencing, care coordination, accurate billing and documentation, and alignment with aged care standards. Applicants should have tertiary nursing or healthcare qualifications, current AHPRA registration, aged care legislation and quality standards knowledge, case management experience, community nursing experience, strong digital literacy and the ability to work independently and in a multidisciplinary team. This is a permanent full time role.
