Signs off on Case Manager notes and treats individuals with mental, emotional and substance abuse problems through individual or group therapy, crisis intervention, advocacy, prevention, education and culturally relevant, non-judgmental counseling and psychotherapy. Duties include intake and assessment, referrals for substance abuse and psychiatric care, risk reduction support, referral coordination, individual and group interventions, assessments and referrals for Home and Community Based Services and harm reduction programs, case file documentation, activity and statistical reporting, team meetings and clinical supervision. Requirements include a current LCSW certification and New York City or New York State license, harm reduction expertise, at least two years of field experience signing off on case notes, teamwork, organization, computer skills and commitment to a multicultural and multiracial environment. Bilingual English and Spanish skills, experience working with drug users and sex workers, and knowledge of electronic health records and billing are preferred.
