Provide case management and problem-solving intervention services to seniors, specifically the Korean and Korean-speaking population. Assess clients, coordinate the agency's home delivery meal service for eligible seniors, maintain complete and accurate case records, and assist with program planning and evaluation. The role requires strong documentation, organizational, communication, and care-planning skills; a degree in nursing, health, social work, gerontology, or a related field, or equivalent experience with older adults; direct social or health care service experience; a valid California driver's license and safe driving record; basic computer skills; and knowledge of Korean, Korean American, and general community resources. Bilingual and biliterate Korean and English is preferred. The position is supervised by the Social Services Coordinator and does not directly supervise staff.
