Remote-first role working with patients across North East London. The Clinical Coach helps patients build health literacy, confidence and self-management skills through structured, evidence-based coaching. Responsibilities include identifying suitable patients, motivating participation, developing personalised care plans, coaching patients to navigate healthcare services, supporting personal health goals, documenting interactions and outcomes, and working with multidisciplinary health, social care and voluntary sector teams. Applicants should be registered with the NMC, HCPC or an equivalent professional body, have experience supporting patients with long-term or complex conditions, and demonstrate strong communication, interpersonal, organisational and digital skills. Knowledge of coaching techniques or motivational interviewing is desirable, with training provided if needed.
