Medical social workers provide counseling to patients and families about in-home health care policies and resources, explain additional treatment options, offer guidance on illness and trauma recovery and their mental and physical consequences, and facilitate communication with the medical team. Services include coordinating meals, transportation, and support groups, assessing the home environment, monitoring and reporting patient progress and changes in medical condition, collaborating on Plans of Care, encouraging self-reliance, and referring patients and families to community resources and support groups.
