Official USCIS Civil Surgeon medical exam. The office fee is $275.00, payable by cash, check or credit card, and includes the exam, blood sample collection, answering questions, coordination of a chest X-ray only if TB blood tests are positive, and completion and signing of the USCIS 693 envelope. Lab tests, chest X-rays and vaccines are not included in the office fee and are the responsibility of the patient or their insurance company. Same day or next day appointments are available, including Saturday and Sunday appointments.
