Access Transit Pass
People experiencing homelessness who are currently experiencing homelessness in San Francisco and have contacted one of the City's Access Points in the last six months may receive a free pass for access to Muni vehicles through the expiration month and year printed on the card. The card can be mailed to a service provider or another location where the passholder receives mail, or picked up at the SFMTA Customer Service Center.

CONNECT Community Service Program
Eligible people experiencing homelessness in San Francisco can receive the option to complete community service at participating nonprofit organizations instead of paying citation fines. There is no enrollment fee. Participants must currently be homeless in San Francisco and have contacted one of the city's access points within the past six months. This is a one-time offer, and late penalties are waived after enrollment.

If you are ONLY submitting a copy of your registration to replace your short-term permit, DO NOT COMPLETE this form.
