Provide relationship-centred care management through a clinical lens for clients in Western Sydney. Responsibilities include care planning, funding and portfolio management, clinical assessments, direct nursing care, risk screening, case conferencing, discharge planning, coordination with clinical and allied health teams, accurate documentation and supporting clients to live independently. Applicants need a Bachelor of Nursing or similar, current AHPRA registration, knowledge of aged care legislation and quality standards, strong clinical knowledge, case management experience, community nursing experience, excellent digital literacy and the ability to work independently and in a multidisciplinary team.
