An evidence based, low intensity one on one multidisciplinary pain management program for people with chronic pain. Includes weekly sessions of up to 3 hours over 8 weeks, delivered by physiotherapists, psychologists and specialist medical physicians. The program covers chronic pain education, relaxation training, pacing, coping skills, stress management, problem solving, physical goal setting, graded functional movement, stretches, resistance and strengthening exercises, cardiovascular exercises, posture and gait education, flare up management and self management strategies. Psychological components are based on CBT principles, physiotherapy is active based and includes reactivation, pain neuroscience education and graded exposure, and specialist medical physician education sessions are conducted twice during the program.
