Register as a new NHS patient within the practice boundary. Registration can be completed online through the NHS Register with a GP surgery service or by downloading, printing and bringing the completed NHS PRF1 form to the surgery. Children under 16 must reside at the same address as the registering parent or guardian, who must be registered or registering at the same time. For children's registrations, provide the front page of the Red Book and copies of all immunisations to date. Patients should ensure they have one month's medication supply from their current GP practice and book a new patient medication review after registering.
