Application to request authorisation to scatter ashes in the Jardin du Souvenir at the new Montreuil cemetery. The form requires an original death certificate issued within the last three months, a cremation certificate, and the contact details of the funeral operator appointed to organise the funeral. A plaque identifying the deceased is required and will be installed by the cemetery service. Installation is free. The cemetery service can also make the plaque for a fee.
