Supports patients with end-stage, life-limiting non-malignant illness, including lung disease, heart disease, renal failure and liver failure. A multidisciplinary team of palliative care consultants, specialist palliative care nurses and social workers provides a holistic approach to care. Carers are invited to attend. The clinic currently operates via telehealth.
