Update your name, address, phone numbers or email address using your NHS account or Patient Access account. For a change of address, you must remain within the surgery's catchment area. Identification is required to change your phone number or email address. Proof of a name change, such as a marriage or civil partnership certificate, deed poll or birth certificate, is required when changing your name.
