Cost-effective, short-term medical and recuperative services for homeless men and women who do not meet the criteria of other healthcare facilities but are too sick to tolerate life in shelters or on the street. Care is provided by a multidisciplinary team for patients with complex conditions. Admission requires an acute medical need, medical and mental stability for management within the program, a need for short-term care, and independence in activities of daily living. Patients are admitted based on referrals from hospitals, shelters, clinics, and other healthcare providers. Patients cannot self-refer.
