Provides holistic initial assessments for new episodes of care referred to Extended Primary Care Teams Planned Care. Includes identifying care needs, equipment ordering, onward referrals and detailed care planning. The team provides community care in patients' own residences, including residential homes, and offers clinical triage for EPCT referrals. Accepts referrals for Newham residents aged 18 and over who meet the criteria for community District Nursing and Therapy services and are registered with a Newham GP. Urgent Planned Care referrals receive a response within 24 hours, while non-urgent referrals are contacted within 72 hours or when clinical input is due.
