Holistic initial assessment for new episodes of care referred to Extended Primary Care Teams planned care. The team identifies care needs, arranges equipment, makes onward referrals, completes detailed care plans and provides clinical triage. Care is provided in the community, including patients' own residences and residential homes. Available to Newham residents aged 18 and over who meet the criteria for community District Nursing and Therapy services and are registered with a Newham GP. Urgent referrals receive a response within 24 hours, while non-urgent referrals are contacted within 72 hours or when clinical input is due.
