Personalised CD19-directed CAR-T cell therapy for adults aged 26 or older with relapsed or refractory B-cell precursor acute lymphoblastic leukaemia. Treatment includes a full consultation and clinical assessment, blood tests, bone marrow assessment, imaging where needed, infectious disease screening, organ function tests, neurological baseline checks, apheresis to collect T cells, possible bridging therapy, lymphodepleting chemotherapy, fractionated cell infusions over several days, around 30 days of inpatient monitoring, and long-term follow-up. Patients must be fit enough for CAR-T therapy, have adequate organ function and no active uncontrolled infection. Live virus vaccines must be avoided from at least six weeks before lymphodepleting chemotherapy until immune recovery.
