Transport for eligible patients to and from hospital who have very specific needs. Patients must be assessed by the Patient Transport Assessment and Booking Team. Eligibility may include needing skilled assistance to transfer to or from a vehicle, having a disability or condition that makes alternative transport difficult or undesirable, a condition that could require skilled assistance during travel, or a disability that could cause risk or embarrassment when using alternative transport.
