Register with the surgery by completing an online registration form, visiting reception between 10am and 6pm to collect a paper form, or downloading and printing a paper registration form. Registration is only accepted from patients experiencing homelessness, sleeping rough or who are vulnerably housed. Check the catchment area before registering. Having your NHS number is helpful.
