Medical care and support for people with long-term or pre-existing chronic illness, including diabetes, heart disease, high blood pressure and asthma. Includes access to individual and group programs, medical appointments in a culturally appropriate setting, referrals to specialist and Allied Health providers, advocacy support, and assistance scheduling appointments with GPs, physiotherapists and dentists. The program supports health assessments, goal setting and development of an easy-to-understand self-management plan. Available to Aboriginal and Torres Strait Islander people living in the Southern Metropolitan Region who have, or are at risk of developing, a chronic condition. Some tests and referrals to specialists and other providers may incur costs.
