Includes pre-appointment liaison with your Endocrinologist to finalise starting doses, management of pump ordering paperwork, a practical training session covering settings, device insertion and dose navigation, and regular clinical check-ins during the first 1 to 2 weeks. Please bring fast-acting insulin, any emailed pre-pump instructions, your current glucose monitor and your GP care plan referral. A valid Chronic Disease Management Plan referral from your GP is required to claim a Medicare rebate. Private health insurance rebates may apply depending on your policy.
