Eligibility must be re-evaluated every 6 months or when financial circumstances change. Discount availability is limited. Rates range from referral to a low-fee or Medicaid facility for households below 100% of the Federal Poverty Level, to 60 to 65% of the full fee at 100 to 200%, 70 to 75% at 200 to 300%, 80% at 300 to 400%, and the full fee above 400%.
