Flexible, community-based support for people aged 16 or over who have a mental health diagnosis that impacts daily life. Support may include practical help at home, referrals to housing and health services, assistance attending appointments and treatments, education and employment goals, community participation, wellbeing support, and independent living skills such as cooking, nutrition, budgeting, shopping, maintaining a home and using public transport. Support workers regularly visit people in their home and/or local community. The program supports people living in the community or leaving hospital and is culturally welcoming to First Nations communities, people from culturally and linguistically diverse backgrounds, and LGBTQIA+ people.
