An in-depth assessment of general health, past illness, medications, mobility and social support. A Practice Nurse takes measurements including blood pressure, height and weight, and may perform other investigations such as an ECG if necessary. Recommendations are discussed and a copy of the assessment is provided. Eligibility includes people aged 45 to 49 at risk of chronic disease, people aged 40 to 49 at high risk of type 2 diabetes, people aged 75 and older, Aboriginal and Torres Strait Islander people aged 55 and over, people with an intellectual disability, and former serving members of the ADF. Medicare-funded eligibility may apply.
