Specialist diabetes support, regular reviews and day to day care. Annual reviews take place in two parts, a data collection appointment and, where appropriate, a follow up care plan appointment. The initial appointment checks blood pressure, weight, urine, feet and wellbeing. Patients should bring an early morning urine sample.
