A home monitoring program for patients after consultation with the St. Joseph's Urban Family Health Team. The package includes a pulse oximeter, batteries, usage guidance, a patient log, alcohol swabs and return supplies. Patients monitor temperature and/or oxygen levels at home, provide readings during virtual visits, receive a follow-up call to confirm successful device use, and return the device once cleared by their clinical team.
