Patients should bring a valid referral from their GP or specialist, a Medicare card and/or pension card, an up-to-date medication list, relevant tests, a recent eye report from an optometrist or ophthalmologist, a blood glucose diary with 5 to 7 days of documented readings, and blood tests no older than 3 months.
