Transport assistance for medical appointments, social events and activities, and shopping. Available for one-off events or regular activities when included in the care plan. Support from a family member or home carer may be possible, subject to discussion with the Care Manager. Transport accessed through a Home Care Package must relate to the goals outlined in the care plan.
