Bespoke support for patients with addiction who are actively using substances or have recently stopped and need a support network to strengthen recovery and avoid relapse. May include bespoke therapy, assistance attending 12-step meetings, coordination with other addiction services, regular urine drug testing and hair drug testing with patient consent, and use of Soberlink alcohol monitoring with patient consent. Detox and hospital admission support may also be discussed where appropriate.
