Children who suffer from emotional trauma, chronic medical conditions, mental disorders, or anxiety may be eligible to join the Health Home program. The program provides care management, community and social resources, health care providers, mental health and substance abuse providers, medications, housing support, social services, medical transportation setup, school related services, community programs, and support groups, all free of charge for Medicaid participants.
