Temporary Case Manager

Under the supervision of the Program Manager and in conjunction with the Case Management Team, provide crisis counseling and case management to adults, particularly those with significant mental health issues, to support their transition from homelessness to a more stable living situation. Act as an access point to substance abuse treatment, medical and community mental health systems in San Francisco. Help participants develop life skills for housing retention, including money management, budgeting and housekeeping. Provide housing retention support, manage a housing assistance fund, facilitate groups, workshops and socialization activities, and conduct regular case management sessions at the office or in clients' hotel rooms. Qualifications include three years of experience working with people experiencing poverty, homelessness, mental health issues, active drug use or related issues, or a relevant bachelor's degree plus two years of recent case management experience. Previous experience with client-centered case management, housing and treatment referrals, harm reduction principles, teamwork, confidentiality, documentation and communication skills are required or preferred as stated in the announcement.
