Deliver high-quality, nurse-led care virtually and in clients' homes. Duties include conducting mental and physical health assessments, providing wound care management, assisting with medication support, developing safety plans, providing clinical leadership in the community, and collaborating with support workers and families. Requirements include current AHPRA registration as a Registered Nurse or Enrolled Nurse, demonstrated clinical experience, current First Aid and CPR or Basic Life Support certification, a valid Australian Driver's Licence and reliable transport, and current National Police Check and NDIS Worker Screening Check.
