Become a provider in the Podiatry Plan network by completing the New Member Questionnaire, submitting the application and required documents through the Provider Portal within 10 business days, and completing peer review. Required documents include a current CV, California State Medical License, DEA Certificate, malpractice certificate with minimum coverage of $1,000,000/$3,000,000, evidence of board certification or examination qualification, ECFMG Certificate if a foreign graduate, current W-9, and current Cyber Certificate. Approved applicants receive a contract to sign.
