Coordinate aged care clients on Support at Home packages, including home visits, care planning, care coordination and management of individualised budgets. Responsibilities include conducting assessments, monitoring clinical risks, maintaining case notes, following up referrals and reviews, building relationships with clients and providers, and promoting services at community events and networking forums. Applicants require AHPRA registration or relevant nursing or allied health qualifications, assessment and case management skills, community-based aged care experience, strong communication and administration skills, and a current driver's licence with access to a registered vehicle. This full-time role is based in Chatswood with a community-based component and flexible work arrangements including potential work from home.
