Care Experts are independent, creative advocates for families navigating caregiving challenges. They use clinical expertise and empathy to improve members' lives and health outcomes through virtual care management. Duties include developing tailored care plans, guiding members through caregiving journeys and healthcare systems, coordinating care with providers and resources, and building compassionate relationships through ongoing support. Requirements include an RN, LSW, LCSW, or care or case management certification, 5+ years of care or case management experience, experience with motivational interviewing or health coaching, strong communication and technology skills, attention to detail, adaptability in a remote environment, and adherence to HIPAA regulations.
